ADMISSION POLICIES AND REQUIREMENTS


A. Qualification for admission

    A high school graduate of any recognized private or public school who:
        •  passed the CLDH-EI Entrance Examination ;
        •  has a certificate of good moral character from the school he graduated from;
        •  is willing to abide by the rules and regulations set by CLDH-EI.

B. Requirements for the Entrance Examination

    1.  For Freshmen:
         a.  Two (2) copies of 2X2 colored pictures
         b.  Original and Xeroxed copy of NSO Authenticated Birth Certificate
         c.  Certificate of Enrollment/Report Card ( from the high school where applicant is presently enrolled or high school card )
         d.  P200 - Examination Fee

    2.  For Transferees:
         a.  Honorable Dismissal & Official Transcript of Records (OTR) / authenticated copy of complete grades for evaluation ;
         b.  Certificate of Good Moral Character
         c.  Two (2) copies of 2X2 colored pictures
         d.  Original and Xeroxed copy of NSO Authenticated Birth Certificate
         e.  P200 - Examination Fee


    3.  For Second Coursers:
         a.  Official Transcript of Records (OTR)
         b.  Certificate of Good Moral Character
         c.  Two (2) copies of 2X2 colored pictures
         d.  Original and Xeroxed copy of NSO Authenticated Birth Certificate
         e.  P350 - Examination Fee

C. Application Procedure

1.  Secure and submit completed application form together with other admission requirements to the Admissions and Guidance office.
2.  Proceed to the Accounting Office for the payment of the entrance examination fee.
3.  Return to the Admissions and Guidance Office for the schedule of Entrance Examination.

(Entrance examinations are scheduled every Monday starting November until May )

D. Enrollment Procedure (for revision to adjust to SchoolBliz system)

    1.  Submit to the registrar's office the following enrollment requirements:
         a. High School Graduates ā€“ Admission stub and Original High School card
         b. Transferees ā€“ Admission Stub, Honorable Dismissal and authenticated copy of grades
         c. Enrollees who are holders of a Bachelor's degree ā€“ Original Transcript of Records
         d. For old students ā€“ Start in step number 2
         e. Certificate of Good Moral Character
         f. 2 pcs. 2X2 colored pictures
         g. 2 pcs. 1X1 colored pictures
         h. Photocopy of birth certificate (NSO copy)
         i. One big brown envelope
         j. One big plastic envelope
         k. 3-self-stamped envelopes
         l. Medical Certificate from the CLDH-EI physician
       m. NSO copy of the marriage certificate for married enrollees.

    2.  Proceed to the advisers assigned by course to secure copy of subject loads and class schedules. Designated areas for advisers
         are announced in the General Bulletin Board located in the lobby fronting the main entrance.

         Requirements before subject loads and class schedules are issued:
           a. For Freshmen ā€“ duly signed admission stub
           b. For transferees ā€“ duly signed admission stub and copy of evaluation sheets
           c. For old students ā€“ clearance and grades from the last semester attended

    3. Proceed to the advising/assessment area for your class schedule and assessment of fees.

    4. Pay the assessed matriculation fees to the cashier in the accounting office.

    5. Proceed to the designated area for issuance of school ID and uniforms.

    6. You are now enrolled as an official student of CLDH-EI. WELCOME!
