BASIC EDUCATION ISSUANCE OF CARDS AND CREDENTIALS

A.ONLINE RELEASING OF CARDS AND OTHER CREDENTIALS
FOR NON-GRADUATING STUDENTS:

1. Only soft copy of form 138/Report Card will be released.
2. The Parent/guardian may claim the card by filling up a google form that will be provided at the Facebook page.
3. The soft copy of card will be released through the email provided on the google form submitted by the parent/guardian.
4. The student may claim their hard copy of form 138/report card once the general community quarantine is lifted or at the start of classes.

FOR GRADUATING STUDENTS:
1. This will only work if the school to be transferred in will allow the students to submit a soft copy of their requirements.
2. A parent/guardian may request the credential/s by filling up a google form posted at the Basic Education Official Page.

3. The parent/guardian will be notified if the request sent will be approved or not. The request will be rejected if:
a. The student has an unsettled balance on his/her account.
b. The student has an existing problem with his/her academic records or documents that should be submitted to the school registrar’s office.
4. If the request is granted, the parent/guardian will be notified for the procedure and date of release for the documents (document processing will take 2 to 3 work days before release).
NOTE: Only 50 granted request per day will be accepted for queuing.

5. The soft copy of the document will be released through the email provided on the google form submitted by the parent/guardian.
6. The parent should provide a request letter from the school before the release of hard copy of documents.

B. ISSUANCE OF CREDENTIALS FOR STUDENTS WHO REQUESTED FOR HARD COPY
PROCESS OF REQUEST

STEP 1: A parent/guardian may request the credential/s by filling up a google form posted at the Basic Education Official Page

PROCESS OF REQUEST
STEP 2.1: The parent/guardian will be notified if the request sent will be approved or not. The request will be rejected if:
a. The student has an unsettled balance on his/her account.
b. The student has an existing problem with his/her academic records or documents that should be submitted to the registrar’s office.

STEP 2.2: If the request is granted, the parent/guardian will be notified for the procedure and date of release for the documents (document processing will take 2 to 3 work days before releasing).
NOTE: Only 50 granted request per day will be accepted for queuing. For the releasing of documents, the person who requested the release should also be the one to claim the document(s).

STEP 3: The requested document may be claimed on the guard house assisted by the Year Level Coordinators. Safety health protocols will be followed.
