Online Appointment System
With the decline of the COVID-19 pandemic and the government’s drive to encourage face-to-face learning and in-person interactions with teachers, classmates, and other school personnel, CLDHEI no longer accepts online appointments. To make an appointment please call (045) 982-5019 or visit our campus at San Pablo, Tarlac City to speak directly to appropriate school personnel. Please note that with the threat of COVID-19 still existing, our school strongly urges the wearing of face masks for its students, faculty & staff, and visitors for the better protection of everyone within the campus.

HOW IT WORKS
Step 1 – Register
- To access the CLDHEI Appointment system, Click here
- You will be directed to the CLDHEI appointment webpage, where you will be asked to input all pertinent information.
Step 2 – Choose your Appointment
- Select the date that fits your schedule and verify the information you have selected.
- Please provide your basic information, then click “submit”.
Step 3 – Appointment confirmation and details
- Please verify your email. To receive the confirmed appointment slip, please input your correct email address.
- You will receive a confirmation email of your scheduled appointment and its details on the email address provided.
- You may print or photo capture your appointment slip, to be presented to the Guard at the CLDHEI entrance along with your school/valid Identification card.
NOTE:
- Please secure your appointment 2 days prior to your date of visit.
- In observance of the social distancing policy and towards our aim to minimize any unnecessary physical contact that could result in the further spread of COVID-19, the CLDHEI management encourages all clients to course all their concerns and inquiries through our telephone numbers and online platforms.
