PROCEDURE FOR ONLINE APPLICATION
(FRESHMEN STUDENTS SY 2022-2023)

1. ACCOMPLISH APPLICATION FORM ONLINE

  1. Download the Application Form for Admission Click here to download

2. SCAN AND MAKE A DIGITAL COPY OF THE FF:

  1. Two pcs of recent 2×2 Colored ID Pictures
  2. Certification of Enrollment from the Adviser or School Head
  3.  Applicant’s SHS ID (front and back)
  4. Official Receipt for the Processing Fee
  5. The applicant’s Covid-19 Vaccination Card
  6. Certified true copy of Form 137 from Grades 9 to 11

3. PAY PROCESSING FEE and SEND PROOF OF PAYMENT VIA EMAIL

(INDICATE PAYMENT OPTIONS HERE)

Take a screenshot of your transaction / payment and send it via email to treasury@cldhei.edu.ph with Subject Email: Processing Fee Payment / Applicant’s Full Name/ 1st Choice Course or Program.

Example: Processing Fee Payment / Juan Dela Cruz / BS Nursing

4. SUBMIT APPLICATION FORM, REQUIREMENTS AND PROOF OF PAYMENT VIA EMAIL

Submit accomplished application form, digital copy of the initial requirements and processing fee payment via email to the Registrar at registrar@cldhei.edu.ph with Subject Email: Application for Admission / Applicant’s Full Name / 1st Choice Course or Program

Example: Application for Admission / Juan Dela Cruz / BS Nursing

5. WAIT FOR ACKNOWLEDGMENT EMAIL FROM REGISTRAR AND SCHEDULE OF EXAM AND INTERVIEW FROM S.A.O.

Reminders:

  1. Secure hard copies of your initial requirements and proof of payment in case they need to be presented for validation purposes.
  2. Accounting Department will send you a reference transaction number and soft copy of your Official Receipt (via email) within 1-2 days from the receipt of your proof of payment. (You may claim the original copy of your Official Receipt when you visit the school).
  3. Immediately inform the Registrar of any changes made to your email or contact details.
  4. Check your Spam or Junk folders as some of the emails get automatically filtered in there.

(FRESHMEN STUDENTS SY 2022-2023)

  1. Application Forms are available at the school gate, or
  2. Download and print the form Click here to download

2. PREPARE THE FOLLOWING REQUIREMENTS:

  1. Two pcs of recent 2×2 Colored ID Pictures Original and photocopies of the ff:
  2. Certification of Enrollment from the Adviser or School Head
  3.  Applicant’s SHS ID (front and back)
  4. Official Receipt for the Processing Fee
  5. The applicant’s Covid-19 Vaccination Card
  6. Certified true copy of Form 137 from Grades 9 to 11

3. SECURE AN APPOINTMENT

Secure an appointment at least 2 days before your intended visit. To set an appointment Click here.

4. PAY THE PROCESSING FEE

On day of your visit, bring all requirements and proceed to the cashier to pay the processing fee. Secure Official Receipt from cashier.

5. SUBMIT ALL DOCUMENTS TO THE REGISTRAR.

The registrar will endorse you to the Student Affairs Office where you will be given an exam and interview schedule.

Reminders:

  1. Don’t forget to bring original and photocopies of your initial requirements in case they need to be presented for validation purposes.
  2. A student is allowed only one (1) companion when visiting the school.
  3. Vaccination cards are required to be presented at the school gate.
  4. Bring your own pen.
  5. Immediately inform the Registrar of any changes made to your email or contact details.
  6. Check your Spam or Junk folders as some of the emails get automatically filtered in there.
