College Enrollment Procedures

COLLEGE FRESHMEN SY 2023-2024

NOTICE! With the decline of the COVID-19 pandemic and in line with the government's drive to promote face-to-face classes and in-person interactions with teachers, classmates, and other school personnel, all applications for admission will now be conducted in person at the CLDHEI campus is San Pablo, Tarlac City. This means that all online admission procedures are no longer applicable. Please proceed to the Admissions and Registration Office at the CLDHEI campus to apply for admission. However, please note that with the threat of COVID-19 still existing, our school strongly urges the wearing of face masks for its students, faculty & staff, and visitors for the better protection of everyone within the campus.

Admission Test and Interview

  1. Fill out APPLICATION FOR ADMISSION form.
    Forms are available at the Admissions & Registration Office
  2. Pay the examination fee at the Cashier who will issue an official receipt (OR) for the amount paid.
  3. Present APPLICATION FOR ADMISSION form, with the required documents, to the Student Affairs Office (SAO)
  4. SAO will issue the stub indicating the date when admission examinations shall be taken
  5. Take the admission examinations and the interview on the date written on the stub
  6. The admission examinations result will be sent to the email address stated in the application form.
    Please note that all courses have limited slots, and once all slots are filled, new applicants will be automatically included on the waitlist.
  7. If qualified, the student is advised to seek "FIT TO STUDY" medical certification from a licensed physician before the date of enrollment.
REQUIREMENTS:
  • 2 pcs 2" X 2" colored ID picture
  • Grade 11 rating
  • Enrollment certificate from adviser or school head
  • Official receipt (OR) of the payment for the examination fee

Registration and Fees Assessment

  1. Present requirements to the Admissions & Registration Office at any day during the scheduled enrolment period
  2. Fill out the STUDENT PERSONAL RECORD form.
    The form will be issued by the Admissions & Registration Office
  3. Read and sign the DATA PRIVACY NOTICE/CONSENT form
  4. Submit the two forms to the Admissions & Registration Office
  5. The Admissions & Registration Office will issue a copy of your PRE-ASSESSMENT form.
    This form shows the subjects you are enrolled in, the number of units f the subjects, and the school fees you need to pay in full or installment.
  6. Proceed to the Cashier to make to your full or down payment in cash.
    Full or partial payment shall be within the scheduled enrollment period.
    NOTE: Delayed payment may result in the forfeiture of your admission and the slot assigned to you may be given to other applicants on the waitlist.
REQUIREMENTS:
  • Medical certificate stating that the student is “FIT TO STUDY”.
    Please bring original copies of the laboratory test results for verification purposes.
  • Photocopy of PSA birth certificate
  • Certificate of good moral character
  • Marriage certificate, if married
  • Two pcs 2" X 2" colored ID picture
  • Two pcs 1" X 1" colored ID picture
  • One pc long plastic envelope

Payment of Tuition and Other School Fees

  1. Present the PRE-ASSESSMENT form to the Cashier
  2. Pay the full amount or (if installment) the required down payment.
    Payments may be in cash or through direct bank deposit, online banking, GCash, or Cebuana Lhuillier.
    Keep your transaction records for reference.
  3. The Cashier shall issue the original copy of the official receipt (OR) for the amount you paid in cash or (if payment is not in cash) once your payment is verified.
  4. Keep your copy of the OR for future reference.
    You are now officially enrolled in CLDHEI!
